"DIA:CONN" is a portmanteau of "DIABETES" and "CONNECTION". It is an integrated platform for optimal diabetes management that enables real-time monitoring by integrating insulin pumps/pens, mobile apps, care web, and various glucose monitoring devices (CGM, SMBG, etc).
Patients can receive support for remote monitoring and care services from Health Care Providers (HCPs) through DIA:CONN. This allows patients to continuously engage in personalized healthcare by analyzing and adjusting real-time information such as their glucose levels and insulin injections by themselves.
